Petition Forms

The Exception Petition form is now available via an online portal. The pdf version of the document is no longer in use.

Users will need to log in via their Northeastern credentials in order to gain access.

Upon login, users will have to select the correct petition for them.

Individual Travelers: The Individual Traveler’s petition is for individuals wishing to complete a study-abroad, research, coop or similar experience in a high risk location.

Group Travelers: This petition is for group programs such as, but not limited to, Dialogues of Civilizations, student group travel, group travel that is part of a Northeastern course, and Alternative Spring Break. The faculty/group leader will complete one petition form for all those in the group.

  • Please Note: Individual participants on a group program do not have to complete the Individual Traveler’s petition; however, participants should be made aware that the group leader is required to complete this process and that the destination(s) is considered high risk per the University’s Policy on International Travel. Group leaders should also ensure they are educating participants on the risks, any mitigation strategies, and support resources outlined in the petition.

Make sure to save any progress made before leaving the site. If a loss in internet connection occurs or the website is closed without clicking the save button, then all information entered will be lost.

Once the form has been submitted, contact Northeastern’s International Safety Office (ISO) to make sure the form and supporting documents have been received.
